Adjustment to “child only” idno configuration to fill blank levels

Description

Adjust the “child only” idno configuration so that if a level is skipped in the creation of a record hierarchy there isn’t a blank section in the indo number. So, for example, a nice consecutive idno span goes from 2019.001 to 2019.001.01 to 2019.001.01.01.

But, if, for example, a sub-series is created directly under a collection it has the idno 2019.001...01.

Intervening blank levels should be filled with 00. So 2019.001..01 would become 2019.001.00.00.01.

All you need to do here is set defaults on the levels. It's not behavior specific to "child_only". I've set these on St. Olaf so you can see how it works.
